Responsibilities

  • Assess physical, psychosocial, and economic needs for transition of care planning to a variety of levels of care.
  • Document, verify, and validate specific data required to monitor and evaluate interventions and outcomes.
  • Interview and collect patient-specified data and perform chart reviews related to admission and discharge.
  • Communicate telephonically and electronically with outpatient providers to enhance the continuum of care.
  • Adhere to the policies, procedures, rules, regulations, and laws of the hospital and all federal and state regulatory bodies.
  • Provide leadership in the coordination of patient-centered care across the continuum, develop a safe discharge plan through collaboration with patients, caregivers, and the multidisciplinary healthcare team to arrange appropriate post-discharge services and optimal transitions in care.
  • Maintain a working knowledge of care management, care coordination changes, utilization review changes, authorization changes, contract changes, and regulatory requirements; serve as an educational resource to all Health System staff regarding utilization review.
  • Drive appropriate DRG length of stay, patient experience, and reimbursement for all patients.
  • Promote patient advocacy in an independent care model.
  • Engage the patient, family/significant other, and interdisciplinary team in the discharge process to identify appropriate post-acute resources and community service follow-ups.
  • Actively participate in areas of expertise to further enhance the role of social work within the hospital.
  • Complete duties required for discharge planning, such as options forms and agency referrals.
  • Perform other duties as assigned or required.

Requirements

  • Bachelor’s degree.
  • Licensed by the state of Maine as an LSW.

Pay

The salary for this role will range from $49,700 to $88,800 annually based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, and certifications.
