Care Manager II - Children's Health Homes - Western Region

Lead member of a team that serves youth and adults in community settings, focusing on reducing emergency room visits, hospitalizations, and decreasing overall Medicaid costs. This role uses assessments, care planning, linkage to services, advocacy, and support to coordinate person-centered and family-driven care.

Responsibilities

• Conduct comprehensive assessments, outreach, and engagement for assigned youth, adults, and families.
• Partner with referral sources to build an interdisciplinary care team supporting the member’s needs.
• Facilitate monthly face-to-face visits with adults, youth, families, and their care team for assigned caseload.
• Perform initial and ongoing assessments to help members achieve goals within program requirements.
• Create, implement, evaluate, and modify individual service plans as part of a multi-disciplinary team.
• Refer members to agency services and community programs (e.g., outpatient counseling, dental, primary care).
• Support members transitioning between systems (e.g., hospitalizations, inpatient stays, housing).
• Establish and maintain relationships with community service providers for referrals and service evaluations.
• Maintain required contact with members, families, and the interdisciplinary team; facilitate team meetings.
• Serve as a liaison between the program and internal/external resources, ensuring information sharing.
• Maintain and update records, forms, reports, and summaries in member files per agency and funder standards.
• Travel across counties within the region to serve members as required.

Requirements

• Bachelor’s degree required.
• Minimum 2 years of experience in human services or a related field supporting youth, adults, and families.
• Unrestricted, valid NYS driver’s license for at least 1 year with a clean driving record and minimum insurance coverage meeting agency standards.
• Children’s Health Home only: Must achieve CANS (Child and Adolescent Needs and Strengths Assessment) certification score of 70 within 3 months of hire and annually thereafter.

Physical Demands & Work Environment

• Variable scheduling, including evenings and weekends.
• Daily physical demands: Sitting (up to 6 hours), occasional standing (up to 2 hours), occasional walking (up to 2.5 hours), driving (several times a week up to 6 hours).
• Continuous balancing (up to 8 hours); occasional bending, stooping, climbing, kneeling, pushing, pulling, reaching, and twisting (up to 2.5 hours).
• Weekly lifting up to 10-15 lbs. daily; manual dexterity required for grasping and fine manipulation (1-2.5 hours).
• Occasional exposure to dust, fumes, gases, chemicals, or smoke.
• May require physical restraint of clients (50–300 lbs.) in “life or limb” situations, with potential repetition.

Pay

$21.40 minimum pay rate, $31.00 maximum pay rate, based on experience.

Schedule

Full-time (40 hours per week), primarily Monday–Friday, 9:00 AM–5:00 PM, with flexibility based on client needs.

Location & Travel

Based out of the 96 W Buffalo St office in Warsaw, NY. Requires use of a personal vehicle for on-the-job travel, with generous mileage reimbursement. Travel may be required in Orleans, Genesee, Livingston, Wyoming counties, and occasionally within Hillside’s Western Region.
