About the role

Structured family caregiving is a caregiving arrangement in which a patient lives with a principal caregiver who provides daily support based on the patient's care needs. The goal is to provide necessary care while fostering the patient’s independence in a home environment, offering a range of care options as their needs evolve. This service provides alternative long-term care for persons who meet nursing facility-level care needs while preserving dignity, self-respect, and privacy.

The Care Manager will be a vital part of our operations in Georgia, performing orientations, assessing new patients, and training caregivers to ensure they provide the best service possible. This role requires fieldwork in the Gwinnett/Lawrenceville, GA area.

Responsibilities

  • Home orientations involving signing of paperwork
  • Initial assessments during orientations
  • Periodic supervisory visits/check-ins for on-care cases
  • Training caregivers and patients as required per case
  • Interacting with caregivers and other stakeholders as needed
  • Engaging in interdisciplinary meetings with care managers, caregivers, and patients
  • Collaborating in care practice and communicating impressions of cases to appropriate staff
  • Critically assessing procedures and implementing improvements for case evaluation, feedback, and caregiver training
  • Wearing multiple hats as operations develop
  • Travel within the state of Georgia (Atlanta Metro Area)

Requirements

  • 2+ years of experience working with frail or elderly disabled adults and caregivers
  • Previous field experience in a medical or case management environment (preferred)
  • Experience with data input, case review, and case note entry; Salesforce experience is a plus
  • Hands-on experience in home care (preferred)
  • Working knowledge of ADL/IADL assessments
  • Valid driver’s license with an insured vehicle for travel within the state
  • Excellent verbal and written communication, organizational, and time management skills
  • Strong critical thinking and problem-solving skills
  • Proficient in Microsoft Office (Word, Excel, Outlook)
  • Excellent training skills

Pay

Base pay for this role ranges from $23.00 to $25.00 per hour at the commencement of your first year of employment. Compensation decisions depend on factors such as skill set, years of relevant experience, education, location, and licensure/certifications.
