Care Manager (Behavioral Health)
Remote position in New Jersey serving members in Passaic and Sussex Counties. Candidates must reside in Passaic, Sussex, or a nearby bordering county. Up to 50% local travel required.
About the Role
Develops, assesses, and facilitates complex care management activities for members with primarily mental and behavioral health needs. The role focuses on delivering high-quality, cost-effective healthcare outcomes through personalized care plans and education for members and their families related to mental health and substance use disorders.
Responsibilities
- Evaluates member needs via phone or in-home visits, assessing available resources and recommending or facilitating care plans for optimal outcomes, including addressing behavioral health and social determinant needs.
- Performs outreach through telephonic, digital, home, or other site visits to assess member needs and collaborate with resources.
- Develops ongoing care plans for members with high acuity, identifying providers, specialists, and community resources, including those for mental health and substance use disorders.
- Coordinates between members, families/caregivers, community resources, and care provider teams to ensure accessible services.
- Monitors care plans and member status for changes in treatment side effects, complications, or clinical symptoms, adjusting recommendations as needed.
- Facilitates care coordination and collaborates with providers or specialists to ensure timely access to care or services.
- Collects, documents, and maintains member information and care management activities to comply with state, federal, and third-party payer regulations.
- Provides education to members and families on procedures, provider instructions, treatment options, referrals, and healthcare benefits, including behavioral health and social determinant needs.
- Offers feedback to leadership on opportunities to improve care and quality delivery cost-effectively.
- Complies with all company policies and standards.
Requirements
- Master’s degree in Behavioral Health, Social Work, or a degree from an Accredited School of Nursing with 2–4 years of related experience.
- Active New Jersey license required.
- Licensed Master’s Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN based on state contract requirements with behavioral health experience.
- For NJ Health Plan Only: Master’s degree in Behavioral Health or Social Work and a Licensed Associate Counselor (LAC) or Licensed Social Worker (LSW) required.
Schedule
Monday–Friday, 9:00 AM–5:00 PM.
Pay
$56,200.00 - $101,000.00 per year. Actual pay adjusted based on skills, experience, education, and other job-related factors permitted by law. Total compensation may include additional incentives.
