About the role
We are seeking a Care Manager – Bachelors’ Level to join our team at Pine Rest. This role is embedded in primary care or other specialty healthcare clinics to provide Medical/Behavioral Health Integration services to patients and/or identified populations.
Responsibilities
- Provides embedded services within designated clinics or primary care office settings.
- Regularly meets with psychiatrist and/or other health care providers to present behavioral health information including formulation of problem, impression and recommendations utilizing summary of medical information, relevant psychiatric symptoms, behavioral observations, history, substance abuse information, and information obtained from collateral contact.
- Provides care management intake and holistic evaluation of social and healthcare needs for identified patients and identified patient population.
- Facilitates effective and efficient communication with diverse healthcare and community partners to ensure holistic health care treatment for patients served; which may be achieved through interdisciplinary team meetings, record review, creating care plans, and informal discussions.
- Provides appropriate therapeutic interventions commensurate with educational background.
- Provides health behavior interventions to enhance physical health outcomes using evidence-based behavioral change strategies (including but not limited to problem solving, motivational interviewing, behavioral activation, etc.)
- May include crisis management, intake and referral for placement of patients within the appropriate levels of care, care coordination with community referral sources, and utilization review for initial placement of patients.
- Ensures electronic medical record documentation and reporting is completed thoroughly and in a timely manner, in accordance with departmental procedures.
- Participates regularly in departmental and staff meetings.
- Participate in quality improvement initiatives as designated and may aid in program and workforce development.
- Maintains all Michigan licensure requirements.
- Buils knowledge of current trends and developments in treatment and counseling through appropriate professional literature, medical workshops, and publications.
- Fulfills continuing education requirements as per PRCMHS departmental policies.
- Participates in education and training programs for persons served, families, or community groups.
- Provides consultation to professionals, hospitals, organizations, or agencies outside of Pine Rest.
- Membership and involvement in local, state, and national professional organizations is encouraged.
- Creates a customer-friendly environment.
Qualifications
- Education, Training, and Experience: Preferred licensures include: LLBSW, LBSW, will consider other relevant degrees. An LSST may be accepted with relevant experience in care management. Obtain and maintain in good standing certification commensurate with treatment provided.

The candidate may be required to have a valid Michigan driver’s license and ability access to access Pine Rest Transportation Vehicles for the performance of job duties. Generalist Approach to care such that clinicians are willing to approach challenges presented by colleagues and work to find solutions to a variety of challenges. Team-Based work skills necessary to relate and communicate effectively and efficiency with a wide variety of internal and external contacts, including but not limited to physicians, advance practice professionals, patients and families, multidisciplinary treatment teams, community service providers, and a variety of other individuals in the sphere of the patient under treatment. Demonstrated ability to establish priorities, meet deadlines, and maintain flexibility in situations involving evolving and changing needs. Verbal and written communication skills which enable the worker to convey clear, thorough, necessary, and timely information to individuals, family, colleagues, supervisory, and management personnel. Additional Telehealth Specific Skills, Knowledge And Abilites Proficient with Microsoft Office 365; demonstrated ability to use the software for written communication and scheduling of calendar (appointments). Ability to perform necessary requirements for video conferencing; turn on computers, open video applications, connect peripheral devices, and perform assessments via video technology. Ability to effectively utilize or learn electronic medical records and referral software such as Great Lakes Health Connect, EPIC Care Link, or Michigan Health Information Network.
