Care Manager

Job Responsibilities

• Assess patient needs and develop individualized care plans
• Cookordinate and schedule medical appointments and procedures
• Monitor patient progress and provide ongoing support
• Collaborate with healthcare professionals to ensure comprehensive care
• Educate patients and families on proper self-care techniques
• Maintain accurate medical documentation and records
• Provide assistance with activities of daily living as needed
• Implement dementia care strategies for patients with memory impairments
• Utilize electronic medical record systems such as Cerner, eClinicalWorks, etc.
• Administer IV infusions and perform spinal taps as required

Qualifications

• Bachelor's degree in Nursing or related field (preferred)
• Previous experience in primary care or medical office setting
• Strong knowledge of physiology and medical terminology
• Familiarity with dementia care techniques and memory care practices
• Proficiency in using electronic medical record systems (Cerner, eClinicalWorks, etc.)
• Excellent communication and interpersonal skills
• Ability to work effectively in a team environment
• Caregiving and empathetic nature
