Care Manager Assistant
Job Description
Perform remote and onsite administrative tasks, including client/staff communication, client information processing, and data entry.
Support the well-being of clients through regular visits, counseling, and implementation of care plans.
Accompany clients to medical appointments to foster effective communication and follow-through.
Serve as an advocate for clients with healthcare providers and maintain ongoing communication with family members.
Participate in counseling families and arranging necessary in-home services or senior housing options.
Manage client documentation, paperwork, and financial matters, including applications for benefits.
Provide support during emergency situations and safeguard clients from potential exploitation and abuse.
Participate in a rotating on-call system to ensure continuous client support.
Position Goals
To provide comprehensive support to Care Managers by effectively managing administrative tasks and client care activities.

Qualifications
- Minimum of three (3) years of related experience in a medical office, health agency setting, or similar role with demonstrated computer competency.
- Ability to drive as needed for client-related tasks.
