About the role

The Case Management Coordinator position is located in the West Region of Ohio, covering counties such as Defiance, Lucas, Paulding, Putnam, Williams, Fulton, Harden, Allen, and Auglaize. The position is full-time and telework-based, requiring up to 50% travel within the assigned region, primarily Monday-Friday from 8am-5pm with flexibility.

Responsibilities

  • Evaluates member needs and eligibility through the use of care management tools and information/data review.
  • Collaborates with case managers, supervisors, Medical Directors, and other health/behavioral health programs to resolve barriers and achieve optimal outcomes.
  • Participates in case conferences to obtain multidisciplinary reviews and ensures quality of care.
  • Escalates quality of care issues through established channels and uses negotiation and influencing skills to secure appropriate services.
  • Empowers members to make independent medical and lifestyle choices, and promotes their active participation in healthcare decisions.
  • Serves as a single point of contact for members, assisting in remediation of care gaps and access issues.
  • Maintains compliance with regulatory and accreditation guidelines, and documents care plans and evaluations accurately.

Requirements

  • At least 2 years of experience in behavioral health, social services, or human services.
  • Experience with personal computers, keyboard, and MS Office Suite applications.
  • Experience in children's mental health, child welfare, developmental disabilities, juvenile justice, or public sector human services.
  • Experience in family systems, community systems, case management, child and family counseling/therapy, child protection, or child development.
  • Experience with Ohio delivery systems, including local community networks and resources.
  • Resides in the West Region of Ohio, including Butler, Hamilton, Clermont, Warren, Montgomery, Allen, Hardin, Defiance, or Lucas.
  • Willing and able to travel up to 50% of the time and attend trainings/meetings in New Albany.
  • Reliable transportation required.
  • Mileage is reimbursed per our company expense reimbursement policy.
  • Willing and able to work beyond core business hours as needed.

Qualifications

  • Bachelor’s degree or master’s degree in behavioral health, human services, health services, or public health.
  • Must reside in the West Region of Ohio.
  • Must have 2+ years of experience in children's mental health, child welfare, developmental disabilities, juvenile justice, or a public sector human services or behavioral health care field.
  • Must have 2+ years of experience in one or more of the following areas: family systems, community systems and resources, case management, child and family counseling/therapy, child protection, or child development.
  • Must have 2+ years of experience with Ohio delivery systems, including local community networks and resources.

The typical pay range for this role is $21.10 - $40.90, with the actual base salary offer depending on experience, education, and other relevant factors. A CVS Health bonus, commission, or short-term incentive program may also be offered.
