Care Coordinator/Patient Care Coordinator

Traditional Home Care is growing! We’re seeking a Patient Care Coordinator to manage schedules, oversee patient care, and recruit compassionate caregivers in a supportive, family-owned environment. We deliver non-medical personal care so clients can live independently at home, helping with dressing, bathing, grooming, and meal prep. Join our team and make a real difference every day!

Responsibilities

• Coordinate caregiver schedules and resolve staffing issues.
• Support patients and families with compassionate communication.
• Monitor care plans, authorizations, and in-home visits.
• Ensure all visits are completed and accurately documented for billing, payroll, and EVV compliance.
• Recruit, interview, and onboard reliable caregivers.
• Build strong relationships with referral sources and grow your patient caseload.

Requirements

• Strong communication and organizational skills.
• Comfortable using Microsoft Office.
• Experience with scheduling systems/EMRs and PA State Aggregators.
• Associate’s Degree (preferred).
• At least two (2) years of prior experience in healthcare, home care, or staffing coordination (required).
• Prior hiring/recruitment experience (required).
