About the Role
AppleGate Recovery is looking for a Care Coordinator to join our dedicated team, providing care management services to individuals in recovery and helping them create personalized care plans for long-term success.
Responsibilities
- Provide care management services to individuals in recovery
- Assist in developing support networks and access to community resources
- Maintain accurate documentation and assessments
- Collaborate with healthcare providers and support organizations
- Engage with the community to connect individuals with valuable resources
Requirements
- High School Diploma/GED required
- Certified Recovery Specialist (CRS) or willingness to obtain within 6 months
- Strong communication and organizational skills
- Passion for helping others achieve their recovery goals
