Overview

Sentara Health Plans is hiring a Care Coordinator for LTSS Waiver in Charlottesville, VA and the surrounding areas!

Job responsibilities

  • Conducts both in-person and telephonic health risk assessments for Medicaid members receiving LTSS waiver services in Charlottesville and the surrounding area.
  • Performs a variety of casework duties and provides case management services to patients, families, and designated caregivers.
  • Develops, participates, and monitors multidisciplinary collaboration of services to patients where appropriate.
  • Affords assistance to adult patients and their families with personal and environmental difficulties associated with medical conditions up to and including at times of terminal illnesses.

Requirements

  • Must possess a degree in Health & Human Services or a related field such as Art Therapy, Behavioral Sciences, Child Development, Cognitive Sciences, Community Mental Health Counseling, Counselor Education, Early Childhood Development, Educational Psychology, Gerontology, Healthcare Administration, Human Development, Human Services, Marriage and Family Therapy, Music Therapy, Nursing, Pharmacy, Psychology, Psychiatry Rehabilitation, Psychology, Rehabilitation Counseling, Social Work, Sociology, Special Education, Speech Therapy, Therapeutic Recreation, or Vocational Rehabilitation.
  • All degrees must be from schools listed as accredited on the U.S. Department of Education College Accreditation database.
  • Certification/Licensure: None required.
  • Long Term Care: 1 year REQUIRED
  • Health Plan: 1 year - preferred
  • Medicaid: preferred
  • Home health, case management, health insurance/health plan experience preferred.

Qualifications

  • Must reside in Charlottesville or within a 30-60 minute drive east or southeast of Charlottesville, including areas such as Keswick, Scottsville, Palmyra, and surrounding communities.

Standard working hours: 8am to 5pm EST, M-F
