Join the team at Koinonia Family Services in Los Angeles (Gardena) working in our Enhanced Care Management (ECM) program as a part-time ECM Care Coordinator. As a Care Coordinator, you'll connect individuals to services and resources tailored to their unique needs in the greater Los Angeles area. Bilingual skills in English and Spanish are strongly preferred.

About the role

Enhanced Care Management is a statewide Medi-Cal benefit for individuals with complex needs. The Care Coordinator (Lead Care Manager) leads assessments, care plan development, and ongoing care management for eligible participants under the guidance of an ECM Supervisor. The program targets foster youth and other at-risk youth populations, but may also include their family members or other adults. You'll use motivational interviewing to engage individuals, helping them manage chronic conditions and remove barriers to health, social services, and behavioral health care.

Responsibilities

  • Conduct assessments of individuals' needs and formulate individualized care plans
  • Educate and enroll clients about services, motivating them to engage
  • Meet with individuals in their homes to facilitate services
  • Oversee and ensure seamless coordination of services, including physical, mental, dental care, and social services
  • Act as the primary liaison between individuals and their healthcare providers and social services
  • Stay informed about community resources and actively refer individuals to appropriate services
  • Maintain regular contact with clients to review and update care plans, and monitor progress and outcomes
  • Advocate for the needs and rights of clients within healthcare and social service settings
  • Provide clients and families with resources and guidance on making informed health and lifestyle choices
  • Document services according to ECM program standards
  • Work closely with interdisciplinary teams, community partners, and stakeholders to enhance service delivery

Requirements

  • A strong desire to help people in need achieve successful outcomes
  • Minimum 2 years of case management (or related) experience
  • Knowledge of community resources
  • Excellent interpersonal and communication skills
  • Comfortable talking to people and motivating them to seek services
  • Ability to collaborate with interdisciplinary teams
  • Experience with high-needs populations
  • Proficiency in electronic health records (a plus)
  • A high degree of integrity, ethics, and confidentiality
  • Demonstrated ability to work independently with minimal supervision
  • Cultural sensitivity and awareness
  • Valid driver’s license, auto insurance, and reliable transportation (driving is an essential job function)

Qualifications

  • High school graduate or GED required
  • Bilingual in English and Spanish (strongly preferred)
  • Prior experience as a community health worker, peer support specialist, medical assistant, or similar role (a plus)

Pay

Starting wage of $22.00 per hour, or higher with prior ECM experience.

Schedule

This is a part-time role initially offering 20–25 hours per week. Scheduled hours may increase or decrease based on caseload and business needs. Shifts are generally during normal business hours and may extend into early evenings based on client needs. Some scheduling flexibility is available to accommodate personal schedules, provided client and business needs are met.

Qualified candidates must complete a criminal background clearance through the California Department of Justice and FBI, as well as a pre-employment physical and TB test.
