Care Coordinator, Embedded (Hillsborough, NJ)
Ennoble Care is a mobile primary care, palliative care, and hospice service provider with patients across New York, New Jersey, Maryland, DC, Virginia, Oklahoma, Kansas, Pennsylvania, Texas, Florida, and Georgia. About the role
This full-time, in-office position is embedded within our client's senior living facility in Hillsborough, NJ. The Care Coordinator ensures Ennoble Care delivers high-quality services by collaborating with clinicians, staff, and patients to achieve healthcare goals and maintain open communication.
Responsibilities
- Complete individualized patient care plans and perform care management and coordination using Ennoble Care’s electronic medical record system
- Maintain frequent contact with patients to provide support, manage compliance with care programs, and improve outcomes
- Document all client communications (verbal or written) accurately
- Communicate with Primary Care Clinicians regarding patient emergent needs, life-threatening episodes, and ensure comprehensive care plans are complete and accurate
- Keep Team Supervisor informed of all issues pertinent to the care plan process and any known or perceived concerns
- Collaborate with cross-organizational teams to meet the needs of patients, family members, and partner facilities
- Become proficient in using technology, including secure email, telephone systems, and electronic medical records
- Adhere to documentation protocols and best practices for daily work logs, escalation of client issues, and internal communications
- Provide excellent customer service, demonstrated by positive feedback from customers and patients
- Contribute as a positive and productive team member
- Operate office equipment such as computers, telephones, and copy machines
Requirements
- Comfortable speaking on the phone for extended periods
- Compassionate and empathetic toward patients, with exceptional customer service skills
- Ability to take accurate notes and document tasks in a timely manner
- Strong multitasking skills to manage multiple patients and workstreams while remaining organized and efficient
- Ability to thrive in a fast-paced environment
- Full-time availability, Monday through Friday, during daytime hours at our client's senior living facility in Hillsborough, NJ
- Proficiency in using computers, including Outlook and other Microsoft Office programs
- Knowledge of basic healthcare terms, conditions, roles, and care principles
- Must pass a drug screen and background check
- Positive attitude, adaptability to change, team-oriented mindset, and willingness to learn new skills continuously
