Care Coordinator Eastern Cuyahoga, Lake, Ashtabula County
About the Position
This position provides case management to families, with the goal of keeping youth in their homes, schools, and communities by coordinating the necessary services for youth with complex behavioral health and multi-system needs within a High-Fidelity Wraparound model. The specific activities of the CME Care Coordinator include regularly meeting with youth and their families, developing child and family centered care plans, monitoring the care plans through regular scheduled multidisciplinary wraparound team meetings, and assisting youth and families/caregivers with developing or enhancing access to resources and natural supports.
Care Coordinator Job Responsibilities
- Regularly engage with youth and families on a community-based level in a culturally competent manner.
- Assess level of care using the CANS assessment.
- Connect youth and families/caregivers to resources and natural support and work collaboratively with wraparound teams.
- Organize wraparound team meetings and maintain documentation of these meetings.
- Link youth and families to specialized clinical resources to meet their treatment needs.
- Provides assistance and support in crisis situations; responds effectively and mobilizes access to resources in crisis situations.
- Works closely with youth and families to develop care plans that incorporate their voice and choice, and regularly monitors and adjusts care plans as necessary.
- Collects outcome information by administering and collecting necessary forms to client and family.
Education and Experience Requirements
- A minimum of three years’ experience in children’s behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral health care field, providing community-based services to children and youth and their family or caregivers.
- Must have a valid driver’s license.
- Must pass a background check.
