Position Responsibilities
- Develops a comprehensive, person-centered Life Plan with the individual and their circle of support, as well as their entire service provider team.
- Commits to a respectful, just, and supportive environment for individuals and coworkers aligning with the company’s commitment to diversity, equity, and inclusion.
- Effectively manage a tiered caseload, while tailoring services to individual needs.
- Completes enrollment and eligibility documentation.
- Mets with individuals in their homes, physician/provider offices, and other public places in order to conduct assessments and provide services.
- Works independently from home, as well as reporting to the office at least once per week but may require more depending on business needs.
Requirements
- Bachelor's degree with 2 years of relevant experience OR a Licensed Registered Nurse with 2 years relevant experience OR Master's degree with 1 year of relevant experience
- Relevant experience consists of providing direct care or support, coordination, and/or advocacy for any person(s) on a regular basis. Examples: Direct Support Professional, Teacher, Paraprofessional, Counselor, Social Worker, Care Coordinator, etc.
- Minimum of a Bachelors degree, or a Licensed RN, is required by OPWDD in NYS
- Valid driver's license
