Position Responsibilities

• Develops a comprehensive, person-centered Life Plan with the individual and their circle of support, as well as their entire service provider team.
• Commits to a respectful, just, and supportive environment for individuals and coworkers aligning with the company’s commitment to diversity, equity, and inclusion.
• Effectively manage a tiered caseload, while tailoring services to individual needs.
• Completes enrollment and eligibility documentation.
• Mets with individuals in their homes, physician/provider offices, and other public places in order to conduct assessments and provide services.
• Works independently from home, as well as reporting to the office at least once per week but may require more depending on business needs.

Requirements

• Bachelor's degree with 2 years of relevant experience OR a Licensed Registered Nurse with 2 years relevant experience OR Master's degree with 1 year of relevant experience
• Relevant experience consists of providing direct care or support, coordination, and/or advocacy for any person(s) on a regular basis. Examples: Direct Support Professional, Teacher, Paraprofessional, Counselor, Social Worker, Care Coordinator, etc.
• Minimum of a Bachelors degree, or a Licensed RN, is required by OPWDD in NYS
• Valid driver's license
