Responsibilities
- Provide independent, person-centered care coordination for adults living with serious mental illness (SMI).
- Conduct comprehensive member assessments and develop care plans that identify member needs and strengths.
- Manage a caseload of approximately 40 members.
- Travel to meet members in their homes or in the community throughout Erie County.
- Connect members to resources and services in the community.
- Collaborate with members’ care teams to ensure proper care is being delivered.
- Problem solve with members around practical obstacles, to meet their needs.
Requirements
- A Bachelor’s degree in a human service related field and two (2) years of experience providing direct services to people with serious mental health, developmental disabilities, or substance use.
- Credentialed Alcoholism and Substance Abuse Counselor (CASAC) and two (2) years of experience providing direct services to people with serious mental health, developmental disabilities, substance use.
- A Bachelor’s degree or higher in ANY field with either: three (3) years of experience, or two (2) years of experience as a HH Care Manager serving the serious mental health or Serious Emotional Disturbance (SED) population.
- A clean, valid, NYS driver’s license, three years of driving experience, and reliable transportation to travel locally on a regular basis required.

Pay
The range of pay for this position is $22.19 - $25.00. Starting pay may be impacted based on experience and education. This position includes a bonus earning opportunity, with an annual max potential of $4,800, based on volume of service visits provided and cash equivalent opt out benefits. Compensation adjustments are evaluated annually.
Schedule
This position has a schedule from 8:00 AM - 5:00 PM but we are flexible and want to work with your schedule as well!
