About The Role Griswold Home Care of Fargo is hiring a Care Coordinator, also called a Client Care Coordinator or Care Manager, to own a group of home care clients from the first phone call through their ongoing care. You welcome new clients, build their care schedules, match them with the right caregivers, and stay close to them and their families so their care is always on track. It is a relationship role with real ownership in the Fargo-Moorhead area, and your pay grows as your client base grows. What makes this role different This is not a flat office job. You earn a competitive base plus an uncapped monthly bonus tied to the hours of care your clients receive. You start with an established group of clients and grow your book from there, so the more clients you bring on and keep happy, the more you earn, with no ceiling. Our strongest coordinators earn $60,000 or more, and there are additional bonuses for developing new referral partners in the community. What you will do Answer phone, email, and text inquiries from prospective clients and educate them on how Griswold home care can help. Meet with clients and their families in the home to assess their needs for in-home care. Admit new clients to service and build their care schedules. Match each client with caregivers who fit their needs and personality. Coordinate and monitor each client's care throughout service, answering questions and adjusting the care plan as needs change. Develop and maintain each client's care plan. Follow up with prospective clients and keep your pipeline moving. Build relationships with local referral sources such as senior communities, clinics, and hospital discharge planners to grow your client base. Take part in a very minimal on-call rotation. Handle other office duties as assigned. What we are looking for Genuine empathy and warmth. You connect easily with seniors and their families and put them at ease during a stressful time. An outgoing, people-first personality. You enjoy meeting people and building relationships, which is how you grow and keep your client base. Strong organization and the ability to manage many clients at once. At least 6 months of general office experience, comfortable with phones, email, and Microsoft Office. Home care, senior care, or healthcare experience is a plus but not required. We will train the right person. A valid driver's license and reliable transportation for in-home visits.
