What You'll Do
- Manage and support a caseload of DDD members
- Conduct home visits and build strong relationships with families
- Coordinate Attendant Care, Habilitation, and Respite services
- Advocate for member needs and ensure services align with PCSPs
- Collaborate with DSPs, families, and support teams
- Monitor service utilization, compliance, and outcomes
Requirements
- Experience in HCBS / DDD / Case Management / DSP roles
- Knowledge of Arizona DDD programs and services
- HIPAA compliance and confidentiality awareness
- Strong computer skills (Google Workspace, Apple preferred)
- Valid driver's license & reliable transportation
- Ability to pass APS & CPS registry checks
