About the role
BHNCare Coordinator I position supports youth and families using the Wraparound care model. Key responsibilities include developing and implementing care plans, coordinating services, assessing needs, facilitating CPT meetings, conducting home visits, monitoring progress, and collaborating with stakeholders.
Responsibilities
- Develop and implement care plans for youth and families
- Engage families and teams to coordinate services
- Assess strengths, needs, and priorities of youth and families
- Facilitate Care Planning Team (CPT) meetings
- Complete required assessments like CANS and CRAFFT
- Cover services and referrals across various agencies
- Conduct home and community-based visits to support family engagement
- Monitor progress and update care plans regularly
- Ensure all documentation, authorizations, and safety plans meet program standards
- Participate in supervision, team meetings, and professional development
- Collaborate with internal and external stakeholders to ensure continuity of care
Requirements
- Bachelor’s degree in a human services-related field or 5+ years of relevant experience
- One year of experience working with youth with Serious Emotional Disturbance (SED)
- CANS certification or willingness to obtain certification
- Bilingual Spanish English Speaker (Required)
- Knowledgeable about local resources and child-serving systems
- Highly organized with strong communication and teamwork skills
- Comfortable using electronic health records and Microsoft Office tools
- Valid driver’s license and reliable transportation
