About the role
The Care Coordinator plays a crucial role in ensuring quality services are provided to eligible participants while maintaining compliance with established program standards.
Responsibilities
- Conduct outreach, intake, enrollment, and orientation activities for eligible participants.
- Establish and maintain a trusting and professional working relationship with participants by providing regular contact.
- Complete comprehensive participant assessments to identify strengths, needs, barriers, and goals.
- Develop and maintain individualized service plans in collaboration with participants.
- Provide case management and non-clinical mentoring services to participants.
- Provide service planning and coordination, resource and referrals, basic needs support, and/or other support services to families on caseload.
- Cover systematic follow-ups for all families.
- Participate in community outreach activities, resource fairs, and partnership meetings.
- Maintain accurate documentation in Salesforce and PEIRS within specified timeframes.
- Cookordinate services with external providers to ensure continuity of care and reduce duplication of services.
- Maintain regular contact with participants through in-person, virtual, telephone, and community-based interactions.
- Record all group and individualized services and activities in a complete, concise, and timely manner utilizing program forms into PEIRS (DFPS) and Salesforce (BCFS) software.
- Meet with supervisor on a bi-weekly basis to evaluate status of caseload.
- Attend all required annual trainings.
- Travel as needed.
- Other job duties as assigned per grant requirements.
Requirements
- Maintain certificates required for the position including but not limited to First Aid and CPR.
- Valid Texas Driver’s license with a clear driving record and reliable transportation.
- Pass a pre-employment drug screen and random drug screens.
- Provide proof or work eligibility status upon request.
- Pass a pre-employment and biennial criminal background and motor vehicle check (fingerprints needed if applicant has lived out of state within the past 5 years).
Qualifications
- Education: Required - High school diploma or equivalent. Preferred - Bachelor’s Degree In a Related Field.
- Experience: Required - With a high school diploma, at least 3 years of professional work experience in a related field. Preferred - With a Bachelor’s degree, at least 1 year of professional work experience in a related field.
Skills
- Respond sensitively and competently to the service population’s cultural and socio-economic characteristics.
- Communicate effectively in writing and verbally in English.
- Work in a fast-paced environment.
- Maintain computer literacy required to meet the responsibilities of the position.
- Work effectively and without intensive supervision both independently and as a member of a multidisciplinary team.
