Care Coordinator – ABA

About the role

The Care Coordinator at Children’s Crisis Treatment Center (CCTC) supports the growing ABA program and ensures that treatment team recommendations are followed in a fast-paced environment. This position involves working with children aged 2-14 and their families who present with crisis and behavioral health issues in a trauma-informed manner.

Responsibilities

• Develop and implement a service coordination plan in conjunction with the treatment team
• Ensure treatment team members receive pertinent information regarding case identification, linkage, and coordination of services for children and families with CBH, other service providers, and schools, within CCTC
• Facilitate and attend interagency/IEP meetings as needed; monitor effectiveness of services and reassess service needs if necessary
• Coordinate scheduling to ensure continued supports are in place and request necessary documentation for upcoming appointments
• Ensure that services are being received as prescribed
• Maintain timely and accurate electronic chart records
• Ensure the completion of necessary documentation, outcome measures, and signatures are obtained on relevant treatment documents and that required documentation is updated within the required timeframe
• Document contact and coordination provided
• Provide transportation as needed
• Participate in internal as well as external trainings, ensuring the completion of CCTC mandatory trainings

Requirements

• A minimum of a Bachelor’s degree in Social Work, Social Science, or a related field from an accredited educational institution
• Ability to provide direct services to children and adolescents with serious emotional disturbances and their families
• Experience in working with community families with multiple problems
