About the role
The Care Coordinator at Children’s Crisis Treatment Center (CCTC) supports the growing ABA program and ensures that treatment team recommendations are followed in a fast-paced environment. This position involves working with children aged 2-14 and their families who present with crisis and behavioral health issues in a trauma-informed manner.
Responsibilities
- Develop and implement a service coordination plan in conjunction with the treatment team
- Ensure treatment team members receive pertinent information regarding case identification, linkage, and coordination of services for children and families with CBH, other service providers, and schools, within CCTC
- Facilitate and attend interagency/IEP meetings as needed; monitor effectiveness of services and reassess service needs if necessary
- Coordinate scheduling to ensure continued supports are in place and request necessary documentation for upcoming appointments
- Ensure that services are being received as prescribed
- Maintain timely and accurate electronic chart records
- Ensure the completion of necessary documentation, outcome measures, and signatures are obtained on relevant treatment documents and that required documentation is updated within the required timeframe
- Document contact and coordination provided
- Provide transportation as needed
- Participate in internal as well as external trainings, ensuring the completion of CCTC mandatory trainings
Requirements
- A minimum of a Bachelor’s degree in Social Work, Social Science, or a related field from an accredited educational institution
- Ability to provide direct services to children and adolescents with serious emotional disturbances and their families
- Experience in working with community families with multiple problems
