About the role
Care Coordinators collaborate with clients and their support systems—including community providers—to ensure holistic health and wellness support.
Responsibilities
- Conduct outreach through various methods to engage individuals with chronic medical conditions, mental health issues, and/or substance use disorders, often co-occurring.
- Perform initial and ongoing comprehensive assessments to identify strengths and needs.
- Prepare and revise care plans reflecting client needs and personal goals, focusing on health and wellness.
- Maintain at least monthly contact with clients—telephonically and face-to-face—providing outreach, engagement, and service planning in the field.
- Advocate for and support clients to access resources that promote wellness, self-management, and reduce emergency room visits and inpatient admissions.
- Monitor and coordinate all care, including medical insurance, linkage to treatment providers, and community resources.
- Collaborate with community providers monthly as part of a multidisciplinary team to ensure goal-directed care planning.
- Provide crisis intervention and follow-up as needed.
- Maintain detailed, timely, and accurate records in an electronic medical record system.
- Coordinate with supervisors, office managers, and outreach teams to ensure accurate caseload status (enrollments, closures, screen-outs).
- Complete required monthly documentation to ensure continuity of clients’ medical insurance and accurate billing.
- Participate in weekly team meetings to share feedback and resource information.
- Report and coordinate daily office and field schedules with team members and supervisors.
- Offer resources and consult on medical, psychosocial, substance use, and social service needs.
- Attend periodic trainings to enhance skills and learn about wellness self-management and best practices.
- Participate in bi-weekly individual supervision to address concerns and improve skill development.
- Maintain agency vehicles, including upkeep, documentation, and gas card when assigned.
- Responsible for agency cell phone, laptop, and associated items.
- Follow program guidelines outlined in the personnel manual.
- Report issues or concerns to program administration regularly or as needed while working in the field.
Other Responsibilities
- Maintain confidentiality at all times.
- Support other staff members during absences or staff shortages.
- Represent the agency at meetings and trainings as required.
- Work a flexible schedule, as work locations and hours may change.
Qualifications
- Bachelor’s degree and two years’ experience in Human Services required.
- New York State driver’s license and access to a vehicle required.

Pay
Competitive hourly rate based on experience.
