Responsibilities

• Provide compassionate and skilled care to patients and their families
• Collaborate with interdisciplinary teams to develop and implement comprehensive care plans
• Document patient progress and communicate effectively with healthcare providers
• Ensure compliance with all regulatory standards and policies

Requirements

• Bachelor's degree in a related field (e.g., nursing, social work)
• Current license to practice in the state(s) where you will be working
• Experience in hospice care or a related field preferred
• Strong communication and interpersonal skills
• Ability to work flexible hours, including evenings and weekends
