About the role

As a Care Companion/Social Worker, you will be a vital part of our Collaborative Care team, providing emotional support, brief therapeutic interventions, and mental health assessments to residents in need.

You will be responsible for:

  • Conducting assessments of residents to identify emotional, cognitive, and psychosocial needs
  • Implementing short-term, solution-focused therapy and supportive interventions to address immediate concerns
  • Documenting visit notes and maintaining detailed records of interactions and interventions after each visit
  • Collaborating with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP

Qualifications

Key qualifications for this role include:

  • Associates, Bachelor's, or Master's degree in Social Work, Psychology, Counseling, or another mental health–related field
  • Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings
  • Strong communication, documentation, and relationship-building skills
  • Minimum 1 year of experience in a similar role

Pay and Schedule

Carease Health offers a flat salary of $7,000 - $9,000 annually, with flexible scheduling of approximately 5.5 hours per week during Monday-Friday, within regular 8-5 hours.
