Care Companion - Casual - Bilingual Spanish

Responsibilities

• Complete program outreach
• Engage members (clients) in service by providing direct care to increase socialization and connect to community and resources
• Provide friendly visits (telephonic and in-person)
• Light household chores
• Personal errands (such as grocery shopping and transportation)
• Document direct care visits of members in service, assuring compliance with program regulations
• Manage relationships with multiple stakeholders in the community
• Provide provision of direct client services, including LSS Healthy Transitions and similar services, under the supervision of a certified staff member

Requirements

• Ability to work well with diverse individuals, families, groups and organizations and demonstrate compassion, patience, flexibility and respect for others
• Provide regular in-home/telephonic visits to members and provide transportation in the community as needed
• Have dependable transportation and be willing to transport members in your vehicle
• Computer skills, bookkeeping, computer spreadsheets and word processing skills required
• Excellent communication and customer service abilities required
• Organizational skills, problem-solving and the ability to work both independently and as a member of a team
• A valid driver’s license, a clean driving record and background checks are required
