Care Coach 1 (contract)

About the role

This role involves visiting Medicaid members in various care settings to assess and evaluate their needs, establish a care plan, and coordinate services.

Responsibilities

• Assess and evaluate member's needs and requirements to establish a member specific care plan.
• Ensure members are receiving services in the least restrictive setting to achieve optimal well-being.
• Plan and implement interventions to meet those needs.
• Coordinate services and monitor the case management plan against the member's personal goals.
• Guide members/families towards appropriate resources and interact with other payer sources, providers, and interdisciplinary teams as needed.

Requirements

• Lives in Charlotte or Lee counties in Florida.
• Bilingual in English and Spanish.
• Has a Bachelor's degree in health and human services field.
• Has 2 or more years of related experience.
• Has knowledge of Microsoft Office Word and Excel.
• Has exceptional verbal and written communication and interpersonal skills.
• Has the ability to use a variety of electronic information applications/software programs.
• Is part of Humana's Tuberculosis (TB) screening program.
• Is passionate about contributing to an organization focused on improving consumer experiences.
• Has the ability to drive and provide proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits.

Preferred Qualifications

• Prior experience with Medicare & Medicaid recipients.
• Experience with electronic case note documentation and documenting in multiple computer applications/systems.
• Experience with health promotion, coaching, and wellness.
• Knowledge of community health and social service agencies and additional community resources.

Additional Requirements

• Must have the ability to provide a high speed DSL or cable modem for a home office.
• A minimum standard speed for optimal performance of 10x1 (10mbs download x 1mbs upload) is required.

Pay

Pay Rate Range: 23 - 27 USD hourly
