- Certifications: CERT‑ARRT‑S (copy needed), BLS‑AHA (copy needed), CERT‑ARDMS‑RDCS (VT) (copy needed)
- Social Security Number required
- Date of Birth required
- Work history: no more than a 90‑day gap in the last year
