About the role
Join Community Health Network, a progressive organization committed to providing exceptional care in Central Indiana and Indianapolis. As part of the Ambulatory Care pharmacy team, you will participate in collaborative drug therapy agreements, managing hypertension, dyslipidemia, cardiovascular risk reduction, antiarrhythmic monitoring, tobacco cessation, anticoagulation service, and transitional care management for cardiology discharge patients.
Responsibilities
- Participate in team-based care under collaborative drug therapy agreements
- Manage hypertension, dyslipidemia, cardiovascular risk reduction, antiarrhythmic monitoring, tobacco cessation, anticoagulation service, and transitional care management for cardiology discharge patients both in-person and virtually
- Provide staff education, guideline/policy development and implementation, and participation in performance improvement initiatives and practice advancement
- Mentor and teach pharmacy students and residents
- Serve as a clinical resource for the healthcare team
Requirements
- Strong communication skills and a collaborative approach to patient care
- Practice at the top of your license utilizing established and growing collaborative drug therapy management agreements
- Meet with patients individually (in-person and remotely) to provide extensive drug and disease state education with a particular focus on patients at high risk for hospitalization or poor health outcome
- Work closely with physicians and health care teams, while providing direct patient care
- Work closely with patients to overcome medication adherence issues
- Able to assist patients in obtaining cost-effective medications with the assistance of medication assistance coordinators
- Able to serve as a drug information resource for the other providers and clinic staff, answering medication therapy questions
- Able to monitor for drug-therapy-related adverse events by performing/ordering the appropriate clinical or laboratory tests
- Able to engage in population health projects to impact the lives of patients we serve
- Have a clear understanding of quality improvement principles
- Able to precept pharmacy students and residents
Qualifications
- Graduate of an accredited college of pharmacy, Bachelor or PharmD; Doctorate degree
- Indiana Pharmacist license
- Successful completion of a PGY1 Pharmacy Residency and PGY2 residency with Cardiology or Ambulatory Care focus is strongly encouraged or a minimum of three (3) years working under a drug therapy agreement in a clinical setting
- Board certification as a Board Certified Pharmacotherapy Specialist, Board Certified Ambulatory Care Pharmacist, Board Certified Cardiology Pharmacist is encouraged and required within 2 years of accepting the position
