The cardiac/pulmonary rehab therapist will collaborate with patients and families to provide safe and effective patient care. Duties include examination, evaluation, diagnosis, planning, intervention, and establishing outcomes. The therapist will analyze data, determine prognosis, establish goals, and implement and modify a plan of care based on reassessment and patient response.

  • Partners with the patient and family
  • Gathers pertinent data
  • Performs examination
  • Determines treatment diagnosis
  • Develops an individualized plan of care through collaboration with the patient and family
  • Analyzes data to identify needs and potential risks
  • Determines prognosis for recovery
  • Establishes individualized goals with patient and/or caregiver
  • Individualizes plan of care considering developmental level and cultural differences
  • Implements and modifies plan of care based on reassessment and patient response
  • Captures appropriate discharge plan
  • Coordinates care and communicates effectively with interdisciplinary team for all aspects of patient care
  • Partners with patients and families to provide skilled intervention
  • Implements plan of care
  • Affirms treatment/plan of care to reflect change in patient status and response to treatment
  • Affirms patient and/or caregiver learning needs and provides appropriate education
  • Documents and bills using acceptable medical terminology
  • Follows professional practice standards
  • Completes documentation in a timely manner per department standards
  • Complies with department billing standards

Qualifications

  • Bachelor's degree in Exercise-related field (Exercise Science or Exercise Physiology)
  • MN Licensure in Physical or Occupational Therapy
  • Basic Life Support (American Heart Association or Red Cross)
  • Advanced Cardiac Life Support (American Heart Assoc)

Preferred qualifications include:

  • Master's or Doctorate degree in Physical Therapy or Occupational Therapy, or an Exercise Related field (i.e. Exercise Science, Exercise physiology)
  • One year experience
  • Certified Exercise Specialist (CES)
  • Registered Clinical Exercise Physiologist (RCEP)
  • Certified Clinical Research Professional (CCRP)

Pay

FTE 0.8, authorized to work 64 hours per pay period.

Schedule

Approximately Monday, Wednesday, Friday, 8:00am - 4:30pm and Tuesday/Thursday, 9:30am - 6:00pm.
