About the Role

The MVH Licensed Clinical Social Worker (LCSW) supports MVH's initiatives toward clinical integration by:

This position provides assessment, intervention, and advocacy for patients with varying levels of mental health conditions. The role requires flexibility, independence, and a strong knowledge of the healthcare system, including community resources to maximize emotional, social, and physical well-being. The LCSW coordinates interventions with primary care providers (PCPs) and other care providers.

Mountain View Hospital and its 29 affiliate clinics serve the Snake River Valley from Pocatello to Rexburg, offering comprehensive care including wound care, urgent care, oncology, neurology, physical therapy, speech therapy, a Level III NICU, and a robust robotic surgery department. Responsibilities

• Complete initial screening of patients referred from primary care providers to determine disposition and needs.
• Employ a wide range of interventions, such as care coordination, risk screening, behavior change, educational information, referrals, and safety planning.
• Offer solutions to roadblocks while implementing programs and monitor high-risk patients within each clinic.
• Ensure all deadlines are met during implementation stages and revise care plans as indicated by patient response to treatment and provider direction.
• Perform patient care responsibilities considering age-specific needs and knowledge of medications and their correct administration.
• Formulate and evaluate teaching plans based on identified patient learning needs, including family in teaching as appropriate.
• Perform treatments and provide services and education to the level of licensure.
• Advocate on behalf of patients and their families to gain access to services and resources.
• Provide provider support in complex cases.
• Treat patients and their families with respect and dignity, addressing psychosocial, cultural, ethnic, and religious/spiritual needs.
• Function as a liaison between administration, patients, physicians, and other healthcare providers.
• Interact professionally with patients/families and involve them in the formation of the plan of care.
• Demonstrate knowledge of growth and development principles over the life span and provide age-appropriate care.
• Interpret data about patient status to identify age-specific needs and provide necessary care.
• Perform all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of errors.
• Communicate appropriately and clearly to managers, co-workers, and physicians.
• Maintain good working relationships within the department and with other departments.
• Act as a resource for less experienced staff, including additional educational duties as assigned.
• Ensure documentation meets current standards and policies.
• Coordinate and supervise patient care by working with outside practices.
• Complete annual education requirements and keep licensure current.
• Maintain patient confidentiality at all times.

• Maintain flexibility in a growing environment and accept new duties as the scope of primary care practices evolves.
• Assist MVH with resources for adults with behavioral health needs, including outpatient therapy, psychopharmacology, partial hospital programs, inpatient programs, and self-management tools.
• Effectively track and document calls, outreach, case reviews, and outcomes of referrals in the EMR system.

Requirements

• Licensed Clinical Social Worker (LCSW).
• 6 months of required experience; 2–5 years of clinical experience preferred.
• Experience with Word, PowerPoint, Excel, Microsoft Office, and various EMR systems.
