Bilingual RN Care Manager - Remote, nationwide
About the role
The Care Manager, Telephonic Nurse 2 plays a crucial role in managing members' health through various strategies and techniques. They identify and resolve barriers to care, monitor patient progress, and collaborate with a multidisciplinary team to manage physical, environmental, and psycho-social health issues.
Responsibilities
- Telephonically assess and create care plans for members with physical, environmental, and psycho-social health issues.
- Avoid barriers to care and ensure patients are moving towards their health goals.
- Coordinate care for acute situations, such as APS and EMS, and assist with safe discharges and post-discharge care.
- Facilitate interactions with appropriate resources to support members' health and wellbeing.
- Assess physical, environmental, and psycho-social health issues and implement strategies to manage these issues.
- Work collaboratively with a multi-disciplinary team to manage care plans and intervene promptly.
Requirements
- Active Registered Nurse (R.N.) license with no disciplinary action.
- Active Compact nursing license and reside in the state that holds your compact license.
- Bilingual in Spanish and English.
- Minimum of 3 years of clinical RN experience.
- Experience managing various adult chronic conditions and developing individualized care plans.
- Proficient in computer systems, including Microsoft and Excel.
- Capable of managing multiple priorities and using multiple computer applications simultaneously.
- Effective telephonic and virtual communication skills.
- Ability to manage a dedicated home office space for privacy and security.
Preferred Qualifications
- BSN or MSN degree in nursing or equivalent.
- Previous experience in care management, including knowledge of complex care management and care management principles.
- Experience with motivational interviewing.
- Experience with MCG or CMS guidelines, assessment, and documentation practices.
- Certification in Case Management (CCM).
Work at Home Requirements
- Internet service with a minimum download speed of 25 Mbps and an upload speed of 10 Mbps.
- Space for a dedicated home office to maintain confidentiality and privacy.
Pay Range
$71,100 - $97,800 per year
