Responsibilities

  • Assess client’s personal, medical, emotional, social, and environmental situation through home, community, and office visits in English and Spanish.
  • Cookinate the establishment of an individual recovery plan with the client, client’s support system, and other care providers in English and Spanish.
  • Complete daily progress notes based on services provided to clients in a timely manner per organizational policies and standards.
  • Complete all necessary documentation related to client care, i.e. IMCANS, LOCUS, tracking forms in the Electronic Medical Record, etc.
  • Recognize and act on opportunities to move clients to appropriate levels of care; provide referrals to additional services as indicated.
  • Assist clients in identifying signs and symptoms of de-compensation, assess for crisis situations and or the need for stabilization through hospitalization.
  • Work collaboratively with clients to identify coping skills to assist in managing symptoms and stressors experienced in English and Spanish.
  • Provide psychoeducation, medication training, and monitoring to clients in English and Spanish according to Trilogy policy.
  • Coordinate with team nurse/pharmacy to ensure client medication accuracy.
  • Documenting in real time on medication administration record (MAR).
  • Aid clients in learning and improving independent living skills; i.e. personal hygiene, housekeeping skills, nutrition, and shopping for food and personal items in English and Spanish.
  • Education and assistance with applying for entitlements; i.e. Supplemental Security Income, Social Security Disability Insurance, Medicare, Medicaid, and LINK in English and Spanish.
  • Understand the representative payeeship process per organization policy and provide individualized client money management services.
  • Accompany and transport clients to important appointments in the community and provide support with issues related to housing, substance use, budgeting, social support, and medication.
  • Advocate on client’s behalf and empower clients to advocate on behalf of themselves when appropriate.
  • Communicate effectively within the team model for a multi-disciplinary approach to client care.
  • Actively participate in team communication via multiple electronic platforms.
  • Engage regularly in daily team meetings to consult and collaborate on shared caseload.
  • Perform on-call and crisis coverage as assigned.

Qualifications

  • Bilingual in Spanish required.
  • Comfortable providing services in Spanish.
  • Must have one of the following requirements:
    • A High School Diploma or GED and two years of supervised clinical experience in a mental health setting.
    • An Associate’s or Bachelor’s degree in any field.
  • Valid Illinois driver’s license with daily access to a well-maintained vehicle with $100,000/$300,000 liability insurance.
  • Ability to work evenings and weekends as directed.
