About the Role
Provide in-person psychotherapy services to the geriatric population in skilled nursing, sub-acute, and assisted living facilities.
Responsibilities
- Conduct individual and group therapy sessions.
- Assess, diagnose, and treat emotional and behavioral conditions.
- Develop tailored treatment plans for patients.
- Collaborate with facility staff and families to support patient care.
- Maintain accurate clinical documentation.
- Monitor and track patient progress.
Requirements
- Active, unrestricted Licensed Professional Counselor (LPC) license.
- Fluent in English and Spanish.
- Proficiency with electronic health records (EHR).
- Strong organizational and communication skills.
- Ability to provide in-person care within healthcare facilities.
- Physical mobility to navigate healthcare settings.
