About the role

Humboldt Park Health is a 200-bed, acute care facility deeply committed to serving the Humboldt Park community since 1894. We provide patient-centered healthcare, focusing on quality of care, and offer a range of services including health education, chronic condition management, and outreach for the elderly.

Responsibilities

  • Complete assessments with patients arriving at the Emergency Department or Ambulatory Clinics
  • Aid in providing community resources and scheduling appointments
  • Facilitate referrals for clinically indicated services outside the primary care clinic
  • Closely coordinate behavioral health care with the patient's primary care provider and mental health providers
  • Encourage patients to engage actively in their health through motivational interviewing and behavioral activation
  • Accurately document patient information, assessments, interventions, and encounters
  • Attend ongoing training and learning related to job position and duties
  • Proactively reach out to patients to encourage active engagement in their health
  • Serve as a resource for participants in the program
  • Develop referral relationships with healthcare providers, social workers, and other community-based service providers
  • Schedule patient appointments
  • Provide outreach to the community
  • Assist in the development of resource materials and maintenance of accurate records and data
  • Complete reports to document activities and provide data for measuring impact
  • Perform other administrative office duties
  • Maintain department orderliness and cleanliness

Requirements

  • Demonstrate ability to collaborate and communicate effectively in a team setting
  • Punctual, reliable, and willing to learn
  • Strong interpersonal skills, communication skills, and confidence in seeking out providers' time to review patient progress
  • Displays high energy, empathy, and organization skills
  • Experience with or ability to learn assessment tools and interventions for behavioral health patients
  • Ability and opportunity to work flexible hours
  • Strongly preferred: Bilingual
