Bilingual Community Health Worker - Inland Empire & Desert Regions (Travel Role)

Key Responsibilities

• Conduct telehealth outreach to members and screen for ECM eligibility
• Submit referrals to health plans and document interactions
• Engage in field outreach to identify and support potential ECM participants
• Build relationships with community partners and assist members in accessing healthcare and social resources

Requirements

• High school diploma required; BA or Master’s in Public Health, Social Work, or related field preferred
• Experience in community outreach, social services, or healthcare navigation preferred
• Valid driver’s license, active auto insurance, and reliable vehicle for field work
• Bilingual in Spanish and English
• Strong communication, organizational, and time management skills
