Bilingual Analyst, Case Management - $5,000 New Hire Bonus – Field, 33603, 33604, or 33612

About the role

Candidates must be located in Hillsborough county, specifically Tampa within 4-5 miles of 33603, 33604, or 33612. This role will require 50-75% travel for face-to-face visits with members in Tampa, FL. Schedule is Monday-Friday, 8:00am-5:00pm, standard business hours.

Responsibilities

• Telephonically and/or face to face assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member's overall wellness.
• Develops a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member's overall wellness through integration.
• Determines appropriate services and supports due to member's health needs; including but not limited to: Prior Authorizations, Coordination with PCP and skilled providers, Condition management information, Medication review, Community resources and supports.

Requirements

• Fluent Bi-lingual (English and Spanish)
• Must reside in or within 4-5 miles of zip code 33603, 33604, or 33612
• Willing and able to travel 50-75% of their time to meet members face to face in Hillsborough County and surrounding areas in FL
• 1+ years of experience in behavioral health or a long-term care setting
• Computer proficiency in Microsoft Word, Excel, and Outlook

Qualifications

• Strongly prefers Field Case Management and discharge planning experience
• Ability to multitask, prioritize and effectively adapt to a fast-paced changing environment
• Effective communication skills, both verbal and written
• Managed care experience

Skills

• Bachelor's Degree required, preferably in Social Services or related field

The Typical Pay Range For This Role Is $21.10 - $40.90. This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
