Job Summary
Perform outreach, community engagement, and administrative support duties under supervision as part of a grant-funded pregnancy and child health initiative.
Job Duties And Responsibilities
- Conduct outreach, education, and care management activities under supervisory guidance.
- Facilitate or assist with Centering Pregnancy and Parenting group sessions to promote health engagement, prenatal education, and family support.
- Affiliate with administrative functions, including scheduling, data entry, maintaining tracking forms, and filing program documentation.
- Educate clients about community and health resources; assist with accessing services as needed.
- Advocate for clients and support development of self-care and problem-solving skills.
- Accompany clients to appointments or community resource sites as appropriate.
- Maintain accurate and timely records of activities, visits, and outcomes.
- Provide feedback on barriers to service accessibility and opportunities for program improvement.
- Support other program-related tasks and administrative duties as assigned.
Job Eligibility Requirements
- An educational background equivalent to a high school diploma or GED.
- One year of experience in a position involving patient or public contact.
- Bilingual (English/Spanish) fluency.
- Strong communication, organizational, and interpersonal skills.
- Ability to work collaboratively with clinical and community partners and independently as needed.
- Proficiency with basic office software (e.g., Microsoft Excel, Word) and data management tools.
