Behavioral Health Therapist – Primary Care

About the role
This position is part of the Collaborative Care workstream in Primary Care and plays an integral role in the care teams by providing high-quality, cost-effective care to patients.
Responsibilities
- Collaborate with other medical professionals in a fast-paced environment.
- Work as a team with the primary care staff and team psychiatrist to help deliver brief interventions for patients in Primary Care.
- Provide short, but intense follow-up.
- Use outcome-based models to track patient improvement.
Requirements
- Master's Degree in Social Work, Counseling, Psychology, Marriage and Family Therapy, or other behavioral health related field.
- Required 2 years of experience providing evidence-based treatment for a variety of behavioral health diagnoses for children, adolescents, or adults.
- Required 5 years of experience providing evidence-based treatment for a variety of behavioral health diagnoses for children, adolescents, or adults.
- Preferred Licensed Clinical Social Worker upon hire.
- Preferred Licensed Professional Counselor upon hire.
- Preferred Licensed Marriage and Family Therapist upon hire.
- Preferred Certified Sex Therapist through the American Association of Sexuality Educators, Counselors, and Therapists (AASECT) within 18 months.
- Preferred Basic Life Support/CPR within 6 months.
- Preferred Substance Abuse Counselor upon hire.
- Preferred Clinical Substance Abuse Counselor upon hire.
Qualifications

- Knowledge of evidence-based treatment modalities.

Pay
This position may be eligible for a $3,000 sign-on bonus.
Schedule
Work Schedule: 100% FTE, days. Shifts are scheduled Monday through Friday, 8am to 5pm. This is a hybrid position. You will work at 1121 Bellwest Blvd in Belleville, WI as well as 1620 Mehta Ln in Fort Atkinson with up to two days of remote work from home.
