Behavioral Health RN, Home Health
Location: Fort Worth, TX – coverage area includes Fort Worth and surrounding communities. Full-time, on-site position with no remote or virtual work.
About the Role
As a Behavioral Health Registered Nurse in Home Health, you will deliver specialized psychiatric and mental-health nursing care to patients with physical and mental illness, late-life depression, and substance-abuse concerns. You will provide direct patient care under physician orders, instruct and supervise LVNs/LPNs and Home Health Aides, and assume responsibility for patient admissions, care plans, care coordination, recertifications, and discharge planning.
This position is paid on a per-visit basis; the salary range of $88,000 to $100,000 reflects estimated annual compensation based on years of experience.
Responsibilities
- Conduct initial and ongoing nursing assessments, provide direct care, and manage the patient’s care plan.
- Perform assessment and reassessment visits, communicate findings and recommended interventions to the physician.
- Notify the physician and other team members of changes in the patient’s condition and needs.
- Document and submit assessment, progress, and discharge notes and required paperwork in a timely manner.
- Manage medical illness and perform psycho-biological interventions.
- Evaluate medications for effectiveness, adverse reactions, side effects, drug allergies, and contraindications.
- Instruct patients on prescribed medication regimens and monitor compliance.
- Administer intramuscular medications.
- Manage situational or other crises.
Requirements
- Current State RN license.
- One of the following qualifications:
- Diploma or AA degree in nursing with at least 2 years of recent (recommended within the last 3 years) inpatient or outpatient psychiatric or mental-health nursing experience, or
- Baccalaureate in nursing with at least 1 year of recent (recommended within the last 3 years) inpatient or outpatient psychiatric or mental-health nursing experience, or
- Master’s degree in psychiatric or mental-health nursing with at least 1 year of recent (recommended within the last 3 years) inpatient or outpatient psychiatric or mental-health nursing experience.
- For agencies providing psychiatric services under the Medicare Home Health Benefit, the Behavioral Health/Psychiatric Home Care Nurse’s resume must be submitted to and approved by Palmetto.
- Current CPR certification.
- Licensed driver with current auto insurance and a vehicle in good working order.
Schedule
Monday through Friday, 8:00 AM to 5:00 PM. No on-call requirement.
