Behavioral Health Provider LCSW(A), LCMHC(A), or LMFT(A).

About the role

We are seeking a Behavioral Health Provider LCSW(A), LCMFT(A) or LCMHC(A) to join our team. This position works under the supervision of the Director of Behavioral Health to provide holistic, whole-person healthcare services.

Responsibilities

• Provides evidence-based assessment, intervention, consultation, and referral services for patients with behavioral and chronic health conditions.
• Works cooperatively with the primary healthcare team to improve patient care and increase team efficiency.
• Formulates and facilitates behavioral health, substance abuse, and chronic disease management plans of care using appropriate treatment modalities, interventions, and available community resources.
• Affords assistance to primary care providers with screening, identification, diagnosis, management, and treatment of behavioral health, substance use, and chronic disease issues.
• Engages in ongoing assessment and quality improvement strategies to support primary care/behavioral health integration.
• Provides consultative services to healthcare team members within defined scopes of care and trains other healthcare professionals as needed.
• Completes required documentation related to patient treatment, payment for services, and organizational operations in a timely manner.
• Maintains professional competence through continuing education and stays informed about community agencies and resources.
• Adheres to professional ethics and maintains a patient-first approach.

Qualifications

• Licensed Clinical Social Worker (LCSW or LCSW-A) in good standing with the Social Work Certification and Licensure Board of North Carolina (or equivalent).
• Master’s Degree.
• Minimum two years’ experience in a public health setting.

Skills

• Knowledge of and adherence to professional Code of Ethics.
• Experience in providing evidence-based assessment, intervention, consultation, and referral services.
• Ability to work collaboratively with a multidisciplinary team.
• Proficiency in managing behavioral health, substance abuse, and chronic disease management plans.
• Strong communication and interpersonal skills.
• Ability to engage in ongoing assessment and quality improvement strategies.
• Knowledge of and ability to liaise with community agencies and resources.
