Behavioral Health Nurse

About the role

The Behavioral Health Nurse is responsible for providing direct and indirect patient care services in the home environment or alternative settings. This includes assessing patients, developing a care plan, performing nursing interventions, educating patients and caregivers, and managing medical illnesses.

Responsibilities

• Makes initial assessment visit utilizing observation/assessment skills and assists in developing the patient’s plan of care in coordination with the patient’s physician/allowed practitioner and Clinical Manager
• Manages medical illness per POC; performs psycho-biological interventions
• Conducts comprehensive medication reviews and ensures patients and their caregivers receive information regarding the patient’s medication regimen
• Provides education to the patient and their family regarding medication administration, actions, side effects, etc.
• Provides written materials for patients and their families
• Evaluates, teaches, and reviews medications and compliance; administers intramuscular (IM) or intravenous (IV) medication
• Manages situational or other crises; performs assessments of potential self-harm or harm to others, and refers to the treating physicians as necessary
• Provides teaching and training activities that require skilled nursing personnel to teach a patient, the patient's family, or caregivers how to manage the treatment regimen; teaches self-care, mental and physical well-being, promotes independence and patient’s rights
