Description

The behavioral health Intake Specialist and Referral Coordinator will be part of a multi-disciplinary team providing comprehensive care programs for clients at Prisma Community Care. The program is designed to empower clients to make healthy choices and live well, regardless of their sexual orientation, gender identity, gender expression, or HIV status.

Essential Functions

  • Address behavioral health needs through psychotherapy for assessment, diagnosis, and treatment of individuals, couples, families, and groups.
  • Complete intake assessments, service planning, and coordinate care by referring to other human services within the organization or the greater Phoenix area.
  • Provide psychotherapy services as needed, including individual, family, and group therapy.
  • Provide crisis intervention as necessary.
  • Develop relationships with community referral sources and maintain a resource database.
  • Facilitate educational presentations within Prisma and the community.
  • Participate in supervision with the Clinical Director or designated supervisor.
  • Perform other administrative duties as assigned.

Requirements

  • Minimum Qualifications: Graduate degree in behavioral health with at least one year of clinical experience or case management experience. Extensive knowledge of mental health and community resources. Full understanding of HIPAA and protecting client confidentiality. Proficient in Microsoft Office and EMR systems.
  • Preferred Qualifications: Active license as LMSW, LAC, LAMFT, or LAAC. One year of case management experience. Knowledge of CLAS standards, HIV/AIDS issues, and community resources. Bilingual (Spanish & English).

Environmental Factors and Physical Requirements

  • Work in a climate-controlled office environment.
  • Exposure to computer screens for prolonged periods.
  • Extensive use of clinical documentation via typing.
  • Regularly walk, sit, stand; use hands, talk, and hear.
  • Occasionally reach with hands and arms.

Compliance Requirements

  • Fingerprint Clearance Card within 90 days of employment.
  • TB test within 30 days of employment.
  • Current flu vaccination within 30 days of employment.
  • Hepatitis-B primary vaccination series initiated within 30 days of employment.
  • CPR/BLS certification.
