Behavioral Health Coordinator Day Shift

Essential Job Functions

• Identifies and accesses resources to optimize effective and efficient patient behavioral care outcomes in collaboration with other members of the multidisciplinary team.
• Provides care coordination following discharge for patients and their families with various systems, including hospitals, schools, department of social services and other community agencies.
• Assists the patient and families with personal and environmental difficulties which predispose the patient towards illness or interferes with obtaining maximum benefits from medical care.
• Maintains knowledge of and relationships with community health and social agencies to expand the behavioral health services available to patients via collaboration or referral.
• Affords the patient and family to understand medical recommendations and provide services planned to restore the patient to optimum social and health adjustments.
• Affords other duties as assigned.

Education & Experience

• Required: Bachelor's in Social Work or related field.

Licensure/Certification/Listing

• Required: BLS (CPR)-American Red Cross or AHA Healthcare Provider.
