Essential Job Functions
- Primary job function is to provide brief clinical intervention to a caseload of primary or specialty care patients per established standards and guidelines.
- Functions include engagement and relationship building, assessment, treatment planning, therapeutic interventions, interdisciplinary coordination, and monitoring referrals.
- Secondary function is to assist with resource location and advocating for benefits.
- Provide brief intervention therapy for clients who are not in need of longer term mental health services or who present in primary care environment who require more immediate attention.
- Interventions include motivational enhancement counseling, adjustment to illness, treatment adherence issues, behavior modifications stress management and psycho-education.
- Provide medical health triage for clients referred by other providers in the primary care setting and provide crisis intervention for clients with more emergent behavioral health needs.
- Effectively collaborate with health care providers and case managers within Clearwater Valley and/or St. Mary's and the larger community to meet the needs of mutual clients including coordinating interdisciplinary case conferences.
- Assist patients with transportation issues that impact treatment adherence.
- Appropriately refer clients to community resources and consistently monitor the outcomes of said referrals.
- Complete documentation of services in a legible manner in accordance with established formats.
- Collaborate with other Clearwater Valley and/or St. Mary's departments i.e., Case Managers, Visiting Nurse Service, Respiratory Therapy etc., to ensure continuity of care and a seamless transition back to primary or specialty care.
Minimum Qualifications
- Master’s Degree in Social Work, Psychology or related Behavior Health; preferred Bachelor’s Degree.
- Minimum 3-5 years previous relevant experience working in a mental health facility or practice is strongly preferred.
- Licensed as an LCSW, LCPC, LSW, BSW or PhD Psychologist.
