At Reema Health, we're dedicated to providing personalized, community-based care for individuals living with serious mental illness and substance use disorder. We seek a driven and self-motivated Behavioral Health Community Guide to join our dynamic team.
Key Responsibilities
- Engage and work alongside diverse clients, focusing on those with severe and persistent mental illness, substance use disorders, and complex medical conditions.
- Reach out to members to enroll them in our care model and provide high-quality support to achieve improved health and wellbeing outcomes.
- Assess member needs, support systems, and behavior to guide them through enrollment in behavioral and mental health services.
- Collaborate with clinicians to help patients connect to care, join clinical visits as needed, and follow up on social needs.
- Coordinate referrals and support appointment scheduling and attendance for members, including transportation if necessary.
- Prepare members for services ending by promoting self-sufficiency and independence.
- Document all work with members accurately and promptly, ensuring thorough and accurate records in Reema’s technology and EHR.
- Use technology-based tools and systems daily, including Google Workspace applications.
- Independently troubleshoot technical issues and lift/carry up to 25 pounds occasionally.
Qualifications
- Bachelor’s degree in Social Work, Human Services, Psychology, Health Sciences, or related field.
- 3+ years of direct experience with the population or community to be served.
- Experience with crisis planning, case management, boundaries, and personal safety.
- Proficiencies in critical thinking, time management, strategizing, and multitasking.
- Strong relationship-building skills and the ability to develop, adapt, and execute outreach and care plans.
- Comfortable working in web-based platforms and apps across various devices.
- Ability to independently troubleshoot technical issues and escalate when needed.
Requirements
- 18 years of age or older.
- Flexible and reliable access to transportation, valid Driver's License, Vehicle Insurance, and a satisfactory driving record.
- Compliance with federal, state, and local regulations regarding patient confidentiality (HIPAA).
- Use of technology-based tools and systems daily.
- Lift and carry up to 25 pounds occasionally.

Pay Range
$50,000 - $55,000 USD per year (Albuquerque, NM)
