Behavioral Health Clinician ( LCSW/LMHC) - NY - Mainly REMOTE

This role is primarily virtual (90%+) with occasional in-person member interactions at our Brooklyn hub.

Licensure Requirement

Candidates must hold an active New York State Licensed Clinical Social Worker (LCSW) or Licensed Mental Health Counselor (LMHC) credential.

Responsibilities

• Provide behavioral health support to members in an in-person and virtual setting.
• Develop member care plans for members when behavioral health interventions are needed.
• Provide clinical education to members on preventative care topics, including behavioral health and mental illness.
• Make appropriate referrals for medical support, social resources, behavioral health, and Substance Use Disorder (SUD) to external and internal providers or internal care team.
• Collaborate with internal care teams to assess and determine need for member escalation or placement in different programs (e.g., lower or higher intensity program).
• Maintain engagement with high-acuity members.

Requirements

• 1-year post Master’s Degree work experience.
• Must complete Cityblock’s medical clearance requirements, including evidence of immunity to MMR, Hepatitis B, Varicella, and a TB screen, or have an approved medical or religious accommodation.

Pay

The expected salary range for this position is $95,000 - $105,000. Final salary will be determined by the candidate’s qualifications, years of experience, skillset, and geographic location.
