Behavioral Health Clinician II Integrated Services

About the role

The Collaborative Care Behavioral Health Consultant functions as a core member of a Collaborative Care team that involves the patient's primary care provider (PCP) and a psychiatric prescriber. The consultant is responsible for coordinating and supporting mental health care within the provider's practice and for coordinating referrals to clinically indicated services outside the practice.

Responsibilities

• Support the treating physician by coordinating treatment, providing proactive follow-up of treatment response, alerting the physician when the patient is not improving, supporting medication management, and facilitating communication with the psychiatric prescriber regarding treatment changes
• Offer brief counseling using evidence-based techniques such as motivational interviewing, behavioral activation, and problem-solving treatment
• Help facilitate changes in treatment if patients are not improving as expected

Qualifications

• Master's degree in Counseling or Social Work, or Doctoral degree in Psychology
• Licensed as LCSW-C or LCPC or Psy.D.
• Behavioral Health care experience required
• Experience with screening for mental health and substance use disorders
• Familiarity with brief, structured intervention techniques
• Experience working with patients that have co-occurring mental health, substance use, and physical health problems
• Experience with evidence-based counseling techniques

• Experience with brief assessment and treatment planning for common mental health and/or substance use disorders
• Working knowledge of differential diagnosis of common mental health/substance use disorders
• Working knowledge of evidence-based psychosocial treatments
• Familiarity with brief, structured intervention techniques
• Basic knowledge of psychopharmacology for common mental health disorders that is within the appropriate scope of practice
• Computer skills required
• Flexibility to work within changing environments in healthcare
• Works well in fast paced environment
• Demonstrate knowledge/skills of behavioral health clinical practice in a variety of settings including completing psychosocial assessments, provision of trauma-informed individual, group, and family therapy services, developing and implementing discharge and aftercare plans, coordinating services for patient/family with community providers including knowledge of community resources, Medicare/Medicaid and commercial insurances, knowledge of state and federal regulations as it pertains to scope of clinical practice in multiple settings, effective critical thinking, professional and effective written and verbal communication, prioritizing work assignments and managing time effectively to complete duties, and working effectively and efficiently in a fast-paced environment
• Ability to communicate and collaborate effectively with both internal and external partners (i.e., behavioral health team, medical team, community stakeholders and patients/families)
• Ability to assess, adapt, and calmly respond to changing and/or crisis environment
• Ability to make independent decisions consistent with current policies, procedures, health care and ethical standards
• Ability to be available to adapt hours to meet the needs of partners

Pay

$35.08 - $52.64
