About the Role

The Behavioral Health Case Manager ensures high-quality behavioral health interventions and services to patients of Apicha’s Primary Care Clinic, focusing on access to and retention in care.

Responsibilities

  • Provide proactive behavioral health case management, including comprehensive assessment, reassessment, and individualized care plan development.
  • Ensure communication with the multidisciplinary care team to promote positive health outcomes for patients.
  • Deliver crisis intervention, stabilization, and brief counseling as needed.
  • Coordinate and link patients with additional services (e.g., specialty care, entitlements, long-term mental health and substance use treatment, community follow-up case management).
  • Report outcomes to the multidisciplinary care team and follow up on referrals.
  • Represent Apicha CHC at community events to promote services and recruit patients, particularly individuals with low income and/or from communities of color.
  • Act as a liaison between patients and the multidisciplinary team to facilitate care coordination and promote referrals.
  • Respond to telephone, email, and referral inquiries in a timely manner.
  • Participate in multidisciplinary care team meetings and continuous quality improvement (CQI) activities.
  • Attend internal and external meetings and trainings as requested.
  • Maintain up-to-date, organized, and legible patient records, including timely completion and submission of forms, reports, and summaries.
  • Ensure accurate documentation and reporting in designated portals to meet all center requirements.
  • Adhere to Apicha CHC policies and procedures, including NYC, NYS, and Federal regulations.

Requirements

  • Advanced degree in mental health counseling or social work.
  • Two (2) years of work experience in a related field (may include verifiable internships and unpaid volunteer work).
  • Licensure as LMHC or LMSW required within three (3) months of hire.
  • UAS-NY certification required within one (1) month of hire.
  • Experience working with target populations strongly preferred, including individuals with serious mental illness, developmental disabilities, substance use disorders (SUDs), HIV/AIDS, chronic illness, Asians and Pacific Islanders, LGBTQ+, and immigrants.
  • Excellent written and verbal communication skills in English.
  • Strong customer service skills to assist in resolving patient and provider concerns.
  • Proficiency in using and adapting to various applications (e.g., Outlook, Word, Excel, PowerPoint, eCW, eICARE, AZARA, HCS, MAPP, Healthix, PSYCKES).
  • Ability to work effectively in both collaborative and individual settings.
  • Ability to multitask and manage multiple projects in a stressful environment.
  • Fluency in Spanish or an Asian language is a plus.
  • Ability to work flexible hours, including some evenings and weekends.
