About the role
This position is responsible for providing direct care to residents' daily activities, overseeing household operations, and maintaining clinical records regarding each resident’s behavior. The position is full-time, 40 hours per week, working 6:45a - 3:15p.
Responsibilities
- Affords assistance with monitoring and maintaining clients' daily household assignments, schedules, and activities.
- Maintains and monitors clients' appointments with outside providers and social activities as determined by job assignment and in accordance with driving policies.
- Monitors and documents clients' activities through hourly check logs, providing specific information and observations of the resident’s needs, inappropriate behaviors, to the clinical team when necessary.
- Ensures the whereabouts of clients while on duty, enforcing rules and program expectations.
- Prepares and sends shift reports at the end of each shift.
- Maintains accurate and complete clinical records nightly and in a timely manner.
- Conducts room and building searches as scheduled to ensure safety.
- Conducts randomized urinalysis and inputs data into assigned lab software for further testing.
- Knows the individualized care plan for residents and provides support to the resident according to the care plan.
- Contributes to the care planning process by providing the clinical team with specific information and observations of the residents’ needs and preferences.
- Handles meals and serves clients to ensure proper portions are given.
- Conducts smoke breaks in accordance with the smoking policy.
- Performs intake assessments with potential clients seeking programming.
- Completes all scheduled tasks, cares for residents and the facility (including maintenance and cleaning).
- Available for backup and emergency shift coverage as needed.
Requirements
- High School Diploma or GED required.
- Medical Assistant Certification or Bachelor's degree in human service field preferred.
- Experience working with and/or training in delivery of behavioral health services preferred.
- At least 23 years of age, valid driver’s license, acceptable driving record, and proof of personal auto insurance policy to be deemed insurable with Harbor’s vehicle insurance carrier for first and second shift employees.
- Third shift employees will not be required to drive.
- Must remain up to date on annual Defensive Driving for 15-Passenger Vans training requirement.
- CPR/First Aid and NCI/CPI Certifications required within 90 days of employment.

- Knowledge of the individualized care plan for residents and provision of support to the resident according to the care plan.
- Responsible for the completion of all scheduled tasks, care of residents and the facility (including maintenance and cleaning).
- Able to work a flexible schedule.
- Participates in training and development programs to enhance knowledge, skills, and personal service networks.
Skills
- Strong communication and interpersonal skills.
- Ability to maintain confidentiality and professionalism.
- Proficiency in Microsoft Office applications.
- Basic computer literacy.
