Job Summary

This position provides Behavioral Health consultation to children, adolescents, adults and families in order to improve psychosocial functioning.

Responsibilities

• Provide comprehensive assessment and diagnosis of Behavioral and Mental Health clients.
• Develop effective treatment plans that include specific goals and timelines.
• Assist clients in achieving established self‑management goals.
• Evaluate crisis situations and apply appropriate interventions or referrals.
• Actively participate in meetings that support an integrated Medical/Mental Health care model.
• Assist in the detection of “at risk” patients and develop plans to prevent further psychological or physical deterioration.
• Assist in the management of chronic disease by working with patients in self‑management of chronic diseases.
• Refer patients for other treatment when objective progress cannot be documented.
• Provide trainings for staff and/or families on Behavioral Health issues as needed.
• Comply with HIPAA, HITECH, and Part II regulations.
• Perform other duties as assigned.

Education and Experience

• Licensed Clinical Social Worker with a current Kentucky license.
• Maintain compliance with licensure board continuing education requirements.
• Minimum of 2 years of direct clinical experience.
• Integrated medical care model experience preferred.

Qualifications

• Computer skills – knowledge of word processing software.
• Excellent working knowledge of behavioral medicine and evidence‑based treatments for mental health and behavioral health conditions.
• Ability to conduct brief patient contacts and make quick, accurate clinical assessments.
• Effective communication and teamwork within an interdisciplinary primary‑care setting serving large numbers of low‑income and minority patients.
• General knowledge of clinical effects and side effects of prescription and illicit drugs used by mental health and behavioral health patients.
• Ability to design and implement clinical pathways and protocols for selected chronic mental health and behavioral health conditions.

Special Knowledge and Skills

• Effective oral and written communication skills.
• Excellent interpersonal skills.
• Intermediate computer literacy.

Language Skills

• Ability to read, analyze, and interpret business periodicals, professional journals, technical procedures, or governmental regulations.
• Ability to write reports, business correspondence, and procedure manuals; present information and respond to questions from managers, patients, employees, and the public.

Mathematical Skills

• Ability to work with concepts such as probability, statistical inference, fractions, percentages, ratios, and proportions in practical situations.

Reasoning Ability

• Ability to solve practical problems and handle a variety of concrete variables where limited standardization exists.
• Ability to interpret instructions presented in written, oral, diagram, or schedule form.

Certificates, Licenses, Registrations

• Licensed Clinical Social Worker in the state of Kentucky.
• Maintain mandatory continuing education requirements.
• Current CPR certification required and must be maintained throughout employment.

Physical Demands

While performing the duties of this job, the incumbent is regularly required to sit, talk, and hear; frequently uses hands and fingers to handle and feel, and hands and arms to reach. The incumbent will also be required to walk, stand, stoop, kneel, crouch, crawl, climb, or balance. Occasionally lifts and/or moves up to 25 pounds. Vision abilities include close and distant vision within normal range and the ability to adjust focus.

Work Environment

The incumbent is occasionally exposed to outside weather conditions. The noise level in the work environment is usually moderate.
